Why do you some people look small when they are dressed in normal cloths compare to when they may have there shirt off etc? My college roommate was buff but most people didn't know till he took off his shirt or wore a tank top, is it a bodyfat thing or what.

Depends how tight the clothes are as well as the muscles that "stick through the shirt". This is why its important to build huge pecs, thick shoulders and traps and thick forearms. I tell some of my clients we need to build the trap/neck area and forearms because when your out at a bar in a medium tshirt or polo that is what people are going to notice.

Well I wear size medium shirts when I probably should be wearing a large and wear alot of polos from Abercrombie and Fitch. So yea.
In a suit and or in long sleeves unless its something like a Nike Pro loose fit shirt which is actually a bit tight on my arms I don't look how I normally do. However aside from Nike shirts and Hoodies, when I go out I never wear sleeves.
